Insurance
At this time I do not accept insurance, but can offer a super bill that clients can submit to their insurance company. Clients can contact their insurance company to obtain information regarding reimbursement rates.

Cancellation and No Show Policy
If you must cancel or reschedule an appointment, Hallie Miller, LCSW requires at least 24 hours notice. If an appointment is cancelled within 24 hours of the appointment time or the client does not show for their appointment, they will be charged a $50.00 fee.
Right to Receive a Good Faith Estimate of Expected Charges
Under the No Surprises Act
Under Section 2799B-6 of the Public Health Service Act, health care providers and healthcare facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing, upon request or at the time of scheduling health care services, to receive a “Good Faith Estimate” of expected charges. You must submit a written request to Hallie Miller, LCSW in person or by email to hallie@halliemiller.com. A good faith estimate will be provided within 3 business days upon request.
